You need to provide the following documents and information:

1. Copy of the main page of your valid passport and driver license;
2. Color passport size photo against a white background;
3. Date of Birth;
4. Sex;
5. Height (cm) and Weight (kg);
6. The exact place of birth;
7. Previous names (in case you have changed them);
8. Mailing address;
9. Phone number;
10. Fax number;
11. Email address;
12. Your fingerprints on the RCMP C-216c form on legal size paper or on a standard fingerprint form from the country of your residence (USD30 additional fee applies if local form been used). The fingerprints form must contain: full name, date of birth and sex of the applicant, the name and address of the police agency, a signature of the official taking the fingerprints;
13. Translation of your fingerprint form into English if done in another language;
